Please take your time and read this agreement very carefully. You will not be allowed to participate in activities at Elevate The Community Inc. without a signed waiver.

All guests participating in an activity at Elevate The Community Inc. courts and/or batting cage must complete and sign the waiver below. Minor participants under 16 years of age must have the waiver completed and signed by their parent, legal guardian, or someone with power of attorney. Upon submission, an email copy of the waiver will be sent to the email address provided in the form. Please retain your email confirmation as it is required to be shown at the registration desk located at the back entrance to gain entry to your upcoming visit at Elevate The Community Inc.

ACKNOWLEDGEMENT, RELEASE AND WAIVER

In consideration of myself and/or my children being permitted to participate in activities and have use of the facilities at the Elevate the Community Centre, I, myself, my heirs, executors, administrators, successors and assigns, do hereby remise and release and forever discharge, waive and save harmless, protect and keep indemnified the Elevate the Community Inc., its affiliated chapters, agents, officials, servants, volunteers, and representatives from and against any and all kinds of actions, claims, costs, and expenses and demands in respect of death, injury, COVID-19, loss or damage to my person or property howsoever caused out of my being permitted to attend at or in any way take part prior to, during or subsequent to the aforesaid classes/activity and notwithstanding that the same may have been contributed to or occasioned by the negligence of any of the aforesaid, their agents, officials, servants, volunteers or representatives. I am aware of the potential risks of participating in sports conducted at Elevate the Community Inc. Centre, especially those risks presented by unprotected side walls.

BY CHECKING HERE, I CERTIFY THAT I HAVE READ THIS ACKNOWLEDGEMENT, RELEASE AND WAIVER FORM CAREFULLY. I FULLY UNDERSTAND ITS CONTENT, AND VOLUNTARILY AGREE TO ITS TERMS AND THAT I, THE PARTICIPANT OR ON BEHALF OF THE MINOR PARTICIPANT, AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE. FURTHERMORE, I UNDERSTAND THAT THE SUBMISSION OF THIS FORM IS THE LEGAL EQUIVALENT OF MY MANUAL SIGNATURE. *
